Street Light Repair Request Form

The Traffic Engineering Division conducts a street light check once a week to locate and repair any malfunctioning street lights.

If you notice a street light that is not working properly, please fill out the Street Light Repair Request Form to report problems.

After you have completed the form, you can do one the following:

1. If you have the full version of Adobe Acrobat (not just Acrobat Reader), you may save the PDF file and email it to the Traffic Engineering Division as an attachment.
2. Print the completed form and mail it to the address given below:
